Horse scratced KZN - Identity dispute

Does anyone know the story behind a horse scratched a few weeks ago(I think it was in KZN ) when the Stipes scratched it because of an Identification problem - I recall an appy was on ?

Scratching: LINEAL FACTOR : Identity discrepancy. (scratched at 16h14). LINEAL FACTOR was 10/1 in the betting at the time of withdrawal.

The Stipendiary Stewards Reported (DA): HIJACKED (K Shea) : The winner. Selected for the taking of specimens for analysis. Trainer D Kannemeyer advised (SA). INJAAZAAT (*M Ntuli). Placed second, Selected for the taking of specimens for analysis. Trainer M Roberts advised (RJ).
In a slow run race APPROACH ME (*D Dillon) and ZIRE SPECIAL (M Yeni) over raced in the early and middle stages.
APPROACH ME (*D Dillon) : Hanging in in the final straight and was difficult to assist in the latter stages.
INJAAZAAT (*M Ntuli) : Was switched out off the heels of HIJACKED (K Shea) to obtain a clear run at approximately the 400 metre mark.
Trainer M Roberts asked to review the patrol videos of this race in the final straight and having done so, took no further action.
In view of the Starter’s report, the Starting Stall certificate of INJAAZAAT has been withdrawn until re-passed tractable with a companion

What does this mean? And what has happened as I can't seem to find anything in the report to explain what occurred or what actions would follow this

A yard bought two horses at the Shongweni Sale and got them mixed up.
